This is an appointment for a healthy athlete in the offseason of their sport looking for a baseline concussion test. This is a battery of tests that are done to establish normal results that can be compared to later after sustaining a suspected concussion or to establish once recovery from a concussion has taken place.

This appointment is for patients following up for management of a concussion that has already been assessed at Form Wellness. This will include further guidance through concussion protocol and may also include physical exertion testing, visual and vestibular testing, or chiropractic treatment. Please dress in light athletic wear with athletic shoes.

This includes a history, physical exam, and report of findings with education and advice based off of current concussion protocols.
