Reserve this care time and Dr. Flatt ND will come to your living room and consult for two or more members of your family. A great choice for moms and kids. Please add a note as to how many individuals I will be reserving time for. This visit fee is set for two existing patients. Additional family members are welcome. Time and fees will be adjusted to reflect this. Separate invoices will be generated for insurance.
Initial consult where Dr. Flatt will take a detailed medical history including: physical/mental/emotional/spiritual/environmental/social. Please see informed trauma care document to privately disclose anything before your visit.
