If you have been involved in a motor vehicle accident, have already been assessed and under care with one of the practitioners, this is the appropriate selection

If you or your child are younger than 18 years of age and have never been to the clinic before, this is the appropriate selection for you.

If your child is less than one year of age, an infant initial is required. A history and physical examination will be covered in this visit as well as a treatment, if necessary.

If you have been involved in a motor vehicle accident, please call the office ahead of time so we can have the appropriate forms ready to be filled out.

If you are new to the clinic, please book an initial consultation. This consultation provides us an opportunity to review the history of your complaint and an overall assessment of your current and past medical history. It also allows time for a physical examination to be performed. Usually a treatment is included in the initial consultation but at the discretion of both the patient and the practitioner.

Most insurance companies will require a prescription from your family doctor in order to get reimbursed for custom-fit orthotics.

If this is a new complaint or you haven’t been into the office in more than six months, a re-examination is required. It allows us the opportunity for additional time to review the complaint or go over any new information that may need to be updated in your health history.

If this is a new complaint or you have not been in to the clinic in greater than six months, a re-examination is required. It allows us the opportunity to review the new complaint and/or go over your medical history since your last visit to the clinic.

If you are over the age of 65 and new to the clinic, please select this option.

If you are a child, student or senior citizen (65 and over) and are a patient of the clinic (have visited the clinic within the past six months), this is the appropriate selection for you

If you are a current patient of the clinic and have been in to see either Dr. Karen or Dr. Samantha for this particular complaint within the past six months, a subsequent treatment is the appropriate selection.
