Entails a detailed history of the presenting complaint, physical examination, and detailed review of medical information. A clinical diagnosis, report of findings, and treatment plan will be established. Treatment will also be included on this visit. Please arrive 5-10 minutes early. Your team or sports organization is undergoing baseline concussion assessment through Complete Concussion Management™ this season. If this is your first time being tested through Complete Concussion Management, please complete the following registration instructions prior to your baseline testing appointment. If you have registered in previous years, simply bring your account number with you to your appointment. Registration Instructions: 1. Visit: network.completeconcussions.com/register 2. Follow steps and enter all information that you can 3. Upon successful registration, you will receive an account number, store this number in a safe place and bring it with you to your appointment 4. At your assigned time, attend your assigned clinic to complete your baseline concussion test!

A history and physical examination will be completed. Physical examination will focus on gait analysis and foot biomechanics. The assessment also includes a 3D scan of both feet to assist in the orthotic fitting process. *If you have healthcare benefits to cover the cost of your orthotics please read your plan and it's requirements. Most benefits allow chiropractors to dispense orthotics, but it is best to double check your plan. To ensure coverage it is best to get a medical doctor's prescription for orthotics that you will submit with the other paper's provided once you receive your custom orthotics.

For concussion patients

This treatment consists of acupuncture and/or dry needling and/or electro-acupuncture.

Follow up appointment. This treatment will consist of chiropractic care plus acupuncture.

Follow-up appointment. This treatment will consist of chiropractic care plus cupping therapy.

This is a follow-up appointment for patients requiring both chiropractic care and laser therapy during their appointment.

This appointment is for patients who have potentially sustained a concussion or have a known concussion. We follow evidence-based protocols for concussion management regarding treatments, return to learn, return to work and return to sports. **If this is a new injury, we will do our best to aim for assessment within 24-48 hours of regular clinic hours.
