If you have seen one of our chiropractors in the past, but haven't had treatment in the last 12 months, please choose this option. A formal re-evaluation is required by the Chiropractic Association of Saskatchewan after 12 months.

If your little one is between under 4 years old, please select this option for their first visit to our chiropractors. Please fill out their paperwork (sent via email) prior to the appointment, and only arrive a couple of minutes early.

If you can't open your mouth, or your jaw is stuck in a position where you can't close completely, we consider this an urgent situation.
