**REQURIED** For patients who have new symptoms or have not been under care for 24 weeks or more. If the doctor determines that as a result of this re-activation visit that chiropractic care can help you, then he will design a thorough treatment plan and communicate that plan to you at your next visit that is referred to as the report of findings. It is not customary to receive a treatment/adjustment on this visit as the doctor needs time to design a proper treatment plan. However, in rare cases, emergency care may be administered by the doctor if deemed absolutely necessary. Investment in your health for this visit is $80.
