This visit allows me to understand your concern, perform a physical examination, and provide treatment on the first visit when appropriate. Approx. 30–45 minutes. Durante esta cita revisaremos tu motivo de consulta, realizaremos una evaluación física y, cuando sea apropiado, se realizará tratamiento en la primera visita. Duración aproximada: 30–45 minutos.

This appointment includes a clinical assessment to determine whether custom orthotics are appropriate for your condition. The visit may include: • Review of health history and current concerns • Biomechanical and gait assessment • 3D foot scan using OOLABS technology • Education on orthotic use, break-in period, and footwear recommendations If custom orthotics are prescribed, payment for the orthotics is collected at this visit and a receipt will be provided. If orthotics are not ordered, an assessment fee of $90 applies.

This appointment is for fitting and adjusting previously ordered custom orthotics. The orthotics will be trimmed as needed to fit properly in the patient’s shoes, and basic instructions will be reviewed. This visit does not include assessment or treatment.

Extended follow up visit for patients with complex complaints, or multiple areas that need attention.

FOR WSIB work related injuries only If you have sustained an injury at work and have reported it to work through the WSIB, select this one. An initial consult will be taken place including history of work injury, forms to fill out, exam and possibly a treatment.

A follow up appointment treatment for infants and children up to 12 years of age.

First visit for a new Youth patient, meaning 0-12 years of age. During this visit we will conduct a history, physical exam, report of findings, render a diagnosis and provide a treatment specific to infants, children and youth.
