Farnoosh Fakoorziba is a Registered Dietitian (RD) with a passion for counselling and education in both English and Farsi. Farnoosh holds a Bachelor in Applied Nutrition. She completed a dietitian internship at Toronto Metropolitan University and has additional certifications in Motivational Interviewing, Trauma Informed Practice and Guiding Clients from Disordered Eating. Farnoosh is a member of the College of Dietitians of Ontario and has over 5 years of experience in seniors' health, as well as in primary care settings supporting clients in chronic disease management including prediabetes, type 2 diabetes, hypertension, dyslipidemia, fatty liver, and weight management.
Farnoosh takes a compassionate and client centred approach in nutrition that focuses on caring for the body, and building a nourishing relationship with food. She focuses on providing simple and practical information that can be applied to everyday life.

The extended (60 minutes) dietitian follow up appointment to help support your nutrition and lifestyle plan. During this session, your dietitian will review your dietary and lifestyle habits, assess your goals and progress, and address any challenges or barriers you may have encountered. The extended visit may include discussing new and ongoing issues that may have occurred. Also, if there are additional questions, and additional support required to help support you (e.g. such as additions and changes to your menu and plan).

As your dietitian, I will be guiding you through the introduction and implementation of the low FODMAP diet as a strategy to reduce IBS symptoms. This is a 3 step diet and the initial consul will focus on step 1 in which a low FODMAP diet is strictly followed for 2-6 weeks. A 7 day low FODMAP meal plan will be sent to you within 48 hours of your initial appointment.

Review of symptom improvement and tolerance to the low FODMAP diet. Step 2 will focus on the re-introduction of FODMAPs in a methodical manner to be completed over 6-8 weeks. The goal of this step is to monitor which FODMAPs are well tolerated and which cause digestive distress. Step 3 will focus on increasing the variety of well tolerated foods into your diet for the long term. Other follow up topics may include; reading nutrition labels, navigating restaurant eating, helpful travel tips and more.

This comprehensive 60-minute virtual consultation with our Nurse Practitioner includes a detailed assessment to determine whether GLP-1 medications may be an appropriate option for you. During your appointment, we will review your health history, current medications, weight loss goals, lifestyle factors, and any previous weight management efforts. We will also review relevant blood work and medical information to assess your eligibility and ensure treatment is safe and appropriate. If indicated, a personalized treatment plan will be discussed, including the potential benefits, risks, side effects, and expectations of GLP-1 therapy. Prescription decisions are made on an individual basis and are not guaranteed. Your practitioner will also send a referral to a nutritionist for nutrition support. This appointment is ideal for individuals seeking evidence-based medical support for weight management and long-term health.
