This intake appointment entails a detailed review of all relevant systems. It may include a physical exam and will consist of questions and conversation clarifying the intake form that must be filled out before you arrive. This time is most productive if you bring along the supplements and medications you are taking as well as any bloodwork or diagnostic interpretations that have been done in the last 2 years.

It is best if I see your face initially (in order to get consent to treatment) then we could move to a phone conversation if preferred. This initial appointment consist of questions and conversation clarifying the intake form that must be filled out before you arrive. If we have time, I will provide you with at least some guidance but I cannot guarantee a complete comprehensive plan during the first visit.
