This is for patients who have already had an initial assessment (within the 8 months - if it has been more than 8 months, please book a re-evaluation) and are requiring follow up treatment.
This includes a thorough history and physical examination after which a diagnosis and treatment plan will be determined and discussed with the patient. Treatment will also be provided on this day.
This appointment is for **CURRENT** patients of the clinic ONLY who have not been for an appointment in 8 months or more. This appointment is not the same as an initial assessment. New patients MUST book an initial assessment appointment. Booking this appointment gives sufficient time for the practitioner to provide a new assessment to determine your needs at this time.
