Billed toMotor vehicle claim
If you have recently been in a motor vehicle accident, you will need to have contacted your adjustor to let them know of your injuries so they can open a claim for this as well as a vehicle accident claim. Please bring your claim number with you for your initial assessment with us. You can either fill out the AB1 form prior to your appointment, or we can fill that out with you at the studio. After your initial assessment is completed, the physiotherapist will then submit the AB2 injury form and recommended treatment plan to the adjustor.
