Follow-Up Appointments provide ongoing evaluation and progression within the chronic disease prevention framework. Conducted by a Registered Kinesiologist, each session includes reassessment of key health and functional indicators, review of adherence to exercise and lifestyle recommendations, and modification of the intervention plan as needed. These appointments ensure continuity of care, optimize outcomes, and support long-term behavior change
