Prior to this appointment you will complete a 3-day food log. During our meeting, we will discuss how small nutritional changes can help to make a big difference in your health. I will provide educational information on a topic related to your health concern and answer any questions you may have. You will also receive a 7-day meal creation to help you get started based on your personal preferences, budget and lifestyle needs. This consultation includes:
o 30-minute consultation
o Review of your current diet
o Presentation on a topic related to your health concern
o 7-day meal creation
o 15-min follow up phone call to answer any questions
