This appointment is for a Government of Ontario Assistive Devices Program (ADP) assessment with a licensed ADP physiotherapist.
The assessment includes:
• Review of medical history and current mobility concerns
• Assessment of strength, balance, posture, and functional mobility
• Determination of eligibility for ADP funding
• Recommendation of an appropriate walker or wheelchair
• Completion and submission of required ADP paperwork
Who should book: Clients who require a walker or wheelchair and are seeking ADP funding.
