Please note, you will be required to pay a deposit of 50% of the amount owing for this appointment. The remaining 50% will be due upon picking up your orthotics.
Also, please note, we are NOT able to directly bill your insurance company for orthotics. You will need to be reimbursed directly from your insurance company and they will only do so once you have physically picked up your orthotics. For more information, please ask!
