Please provide a copy of your Physiotherapy Referral from Vancouver Coastal Health UBC Ortho Recon Program. Please adhere to the suggested treatment schedule as detailed on the form.
There is no cost up front for the patient, unless you decide to use additional sessions outside of the stipulated treatment frequency
