Please bring your photo ID and arrive 10-15 minutes early to fill out the intake form & consent form (available online).
Contact us at info@procarebc.com or 604-401-6670 to clarify if we offer direct billing services for your insurance. There may be unforeseen circumstances related to your insurance coverage where we must collect the full payment for your visit.
We cannot do direct billing for secondary insurance, receipt will be provided for you to submit to your secondary insurance.

Cancellation Policy :
We require 24 hours notice for cancellations.
Late Cancellations made within 24 hours of your appointment are subject to 50% of the total cost of your appointment.
No-Shows (no communication received by the clinic via email, text, or phone call prior to your appointment) are subject to the full cost of your appointment. These fees will be charged to you and cannot be billed to insurance. Thank you for your understanding and cooperation.
