Physiotherapy Initial Assessment is designed to evaluate and address the musculoskeletal health needs of patients within the Primary Care Network (PCN) Program. The PCN certificate grants 3 physiotherapy sessions geared to providing comprehensive care by integrating various healthcare professionals (such as physiotherapists, physicians, nurse practitioners, and other allied health professionals) to address a wide range of health concerns in a patient-centered and holistic way.
The goal of the PCN Physiotherapy Initial Assessment is to identify physical health issues, provide a diagnosis (if appropriate), determine the underlying causes of symptoms, and create a treatment plan tailored to the patient's specific needs. This assessment is integral to ensuring that patients receive appropriate and timely physiotherapy services, particularly those with musculoskeletal pain or functional impairments that impact their daily activities.
