This form should be filled in for all paediatric patients (0-14 yrs.) who are booking their first appointment. This form needs to get reviewed and updated annually.
Patients that have a distressing condition should consider scheduling follow-up appointments right away (typically 25 min. for babies and tots, and 40-min. for older children, once a week, X3 weeks, with a follow-up one month later).
