This is a dual-appointment package starting with the Initial Interview. Upon booking, a link will be sent via email to Nutri-Q where an Initial Intake form, Food and Mood Journal, and Nutritional Assessment Questionnaire must be completed. At our first online appt I will ask a series of clarifying questions in line with the submitted paperwork to formulate a full picture of health concerns and goals. At the end, we will schedule our next appt time for the Recommendations Visit where I will present an individualized protocol and finalize it with your input. These are two separate appts approximately one week apart. Please book at least one week in advance.
