This appointment is for the exam for custom made orthotics. There is a $150.00 non-refundable deposit for ordering custom orthotics, the remaining balance to a total of $425.00 (or more depending on specialty orders) will be paid when the orthotics are dispensed.
PLEASE NOTE: Some insurance companies require a prescription (dated before your appointment) from your medical doctor for reimbursement.
