TRANSFER Patient: Consultation/RE-EXAM & Treatment
PRIOR to this Transfer appointment -- Patients MUST CALL our office. If you are currently under care of a NUCCA practitioner (or have been in the past 5 years) and wish to transfer to our office, we hope to provide you with the best care and continuity possible. A signed consent/release form is required to allow acquisition of your records (e.g. exam forms, X-rays, charting notes) from your current/previous NUCCA provider. If your information is older than 5 years or have not received care within same time period, you may need to be scheduled as a new patient for a more involved intake, exam and new analysis of imaging.
(Note: Records for Patients of Dr. Hasick have already been transferred, therefore no further action is needed).
