Chiropractic New Patient Examination & Preliminary Upper Cervical Radiographs

Current Chiropractic Patients - follow-up treatments are to help progress, maintain or improve upon your initial correction. Last visit must be no more than 1 year ago WITHOUT any significant trauma, falls, motor vehicle collision or surgery since. Please call the clinic if you are not sure how to book.

A more thorough re-assessment with possible changes to your care plan due to recent surgery, injury, stalled treatment progression or 1 (+) year since your previous exam/treatment in our office. Includes treatment. Based on the findings, x-ray fees may be applied to this visit (time permitting).

For patients (12yrs and younger) currently under chiropractic care in our office.

For patients (12yrs and younger): chiropractic treatment with additional time to provide extremity-focused adjustments, focused soft tissue release work and/or application of kinesio tape for added joint support.

Thorough history and examination will be conducted to assess spinal condition and requirements for treatment. Different booking procedures are necessary for children ages 6-15.

Dr. Hunter's EXISTING PRE-NATAL patients specifically needing functional pre-natal chiropractic treatment while expecting. *Pregnant NEW Patients, please call the clinic to schedule your appointment.

A thorough review of care to date, usually after initial 6-8 weeks of care, including followup sEMG scan and/or digital posture analysis, chiropractic review and recommendations for future supportive care. Additional x-rays, if required, will be conducted in lieu of other scans/tests.

PRIOR to this Transfer appointment -- Patients MUST CALL our office. If you are currently under care of a NUCCA practitioner (or have been in the past 5 years) and wish to transfer to our office, we hope to provide you with the best care and continuity possible. A signed consent/release form is required to allow acquisition of your records (e.g. exam forms, X-rays, charting notes) from your current/previous NUCCA provider. If your information is older than 5 years or have not received care within same time period, you may need to be scheduled as a new patient for a more involved intake, exam and new analysis of imaging. (Note: Records for Patients of Dr. Hasick have already been transferred, therefore no further action is needed).
