After booking: We kindly remind you to please bring your motor vehicle insurance information and private health insurance information with you (or email it to us before hand).
Please wear clothing that you feel comfortable moving in. If this is an upper extremity injury, please wear a t-shirt or tank top. If this is a lower extremity injury please bring shorts with you.
Arrive 15 minutes before your scheduled appointment time.
