This physiotherapy assessment type is reserved for those seeking treatment after a motor vehicle accident and are submitting a Motor Vehicle Insurance Claim for physiotherapy treatment (or already have an active claim). After booking: We kindly remind you to please bring your motor vehicle insurance information and private health insurance information with you (or email it to us before hand). Please wear clothing that you feel comfortable moving in. If this is an upper extremity injury, please wear a t-shirt or tank top. If this is a lower extremity injury please bring shorts with you. Arrive 15 minutes before your scheduled appointment time. *Please note that a cancellation fee of $50 will be charged to your account should you change/cancel your appointment within 24 hours of the scheduled time. This will be charged to the credit card on file.*
