Please book this appointment if it is the first time you are seeing Amanda or Elissa with an active ICBC claim.
Please be sure to fill out the online intake form and consent forms. Please provide your claim number and adjustor name and phone number.You may provide your extended health information, as we do direct billing to a variety of extended health plans.
If you have not been officially approved from ICBC for massage therapy by our office by the time of your appointment, we will charge you the regular private rate for that appointment. You can keep your receipt and submit it for reimbursement by your private extended health plan.
