An Initial Assessment is reserved for first-time patients. This consultation allows us time to go over your intake form, perform any necessary physical exams, discuss any diagnostic testing required, and initiate your treatment plan. This is a telehealth visit (secure video call or telephone).
When booking a telehealth visit, please indicate if you would prefer a video call or phone call for your visit in the booking note. If you do not indicate your preference, it will be assumed that you are meeting over video call. If choosing phone call, Dr. Sam will be in touch with you at the time of your visit.
