A reassessment appointment is reserved for existing patients who have not seen Dr. Sam for 1 year (or longer). This visit is virtual (video or phone). If you have not seen Dr. Sam in over 3 years, you will need an Initial Assessment.
When booking a telehealth visit, please indicate if you would prefer a video call or phone call for your visit in the booking note. If you do not indicate your preference, it will be assumed that you are meeting over video call. If choosing phone call, Dr. Sam will be in touch with you at the time of your visit.
