Follow-up visits are available for patients after an Initial Assessment. If it has been at least 6 months since your last visit, we strongly recommend booking a 45-minute follow-up visit to ensure there is enough time to discuss your concerns. If it has been 12+ months since your last visit, you will require a 60-minute Reassessment visit (in-person or online). If it has been more than 3 years since your last visit, you will need an Initial Assessment (in-person or online).
When booking a telehealth visit, please indicate if you would prefer a video call or phone call for your visit in the booking note.
If you do not indicate your preference, it will be assumed that you are meeting over video call. If choosing phone call, Dr. Sam will be in touch with you at the time of your visit.
