Assessment: Falls Prevention, Home Safety, & Equipment

This assessment is targeted towards clients experiencing challenges with balance, mobility, or environmental hazards that increase the risk of falls, with the goal of promoting safety and independence at home. This assessment can also be for clients requiring home equipment or support for a safe and effective hospital discharge, ensuring independence and safety in daily activities. This booking is completed in the home environment and includes an assessment of the home and the client(s) living there (if applicable). Verbal recommendations will be provided during the visit on ways to improve home safety, reduce risk of falls, any relevant equipment options, etc. Please contact us to obtain a travel quote to the home address. No report or written summary will be provided. However, if written recommendations are requested instead of verbal, a Summary Report can be added. Please contact us for further details.
